The Odyssey Institute High School Athletics

PLEASE MAKE SURE TO READ ALL OF THIS INFORMATION PRIOR TO STARTING THE REGISTRATION.

Registrations must be submitted and approved by the deadlines sent out in the registration emails. Notifications will be sent out on Managebac to scholars, and emails will be sent to all parents at the High School. We will also send out notifications via social media so make sure to follow us on Facebook, Instagram, Twitter, and YouTube @GOMINOTAURS.

 

• We encourage you to register for all of the sports you're wanting your athlete to participate in during the entire school year. You will only be able to select one sport per season.  It is easy for us to change a sport later on or remove your athlete from a sport.

 

• Registration should take approximately 10-15 minutes per athlete to complete once you have all the required documents completed. 

 

• Do not start and stop your registration. The system will not save your information!

 

• Once your registration is completed you will receive a "submitted" email as a receipt for your submission. This does not mean your athlete is approved to participate. We will contact you if there are corrections needed. 

 

BEFORE YOU START THIS REGISTRATION

ONLY BEGIN REGISTRATION WHEN YOU HAVE PLENTY OF TIME TO COMPLETE IN ONE SITTING AND YOU HAVE THE INFORMATION/DOCUMENTS LISTED BELOW READY FOR UPLOAD:

 

1. 2024-25 AIA PHYSICAL FORM:

• We will only be able to accept 2024-25 AIA Physical Forms (Unless your physical was completed on or after March 1st, 2024). You will be responsible for uploading this. 
• Forms must be filled out completely and all signatures must be on the forms. 
• All physicals completed on a different year's form will not be accepted.
• There are 6 total pages  AND 2 AIA waiver forms to this physical and all pages must be uploaded to your athlete's registration.

 

2. HEALTH INSURANCE INFORMATION

Athletes must be insured to be able to participate in athletics at Odyssey. You can now take a photo of the front and back of your insurance card or manually enter the information.

 

3. EMERGENCY CONTACT INFORMATION

 Please have this information ready to enter prior to starting the registration process.

  

4. AIA REQUIRED BRAIN BOOK & OPIOID COURSES

(THESE ARE 2 SEPARATE COURSES-IF YOU HAVE COMPLETED THEM BEFORE YOU DO NOT NEED TO DO THEM AGAIN) 

COURSES COMPLETED IN JUNIOR HIGH DO NOT COUNT AND WILL NEED TO BE REDONE.

Follow these steps to complete the two separate courses:

1. Go to https://academy.azpreps365.com/

2. Click register or login as a student on the page.

3. Fill out the account creation information and click "THE ODYSSEY INSTITUTE".

4. Select the sport you are taking the test for and also click any additional sports you may participate in. Then click register.

5. Once registration is complete log in if you do not have an account already.

6. Complete Brain Book Course (ONLY NEEDS TO BE COMPLETED ONCE)

7. Complete Opioid Course (ONLY NEEDS TO BE COMPLETED ONCE)

8. Athletes must answer all of the questions as they go from page to page through the course. (Athletes must pass with at least an 80% or must retake the quiz.)

 

YOU DO NOT NEED TO UPLOAD THE CERTIFICATE ANYMORE. THE ATHLETIC DEPARTMENT STAFF WILL CHECK THE COMPLETION OF EACH COURSE WHILE APPROVING YOUR CHILD'S REGISTRATION. 

 

ATHLETIC FEE INFORMATION:

An invoice will be created and sent to you once the team rosters are finalized. Payments are not being accepted during the registration process.
