-If you have played a sport this school year you will still need to complete this waiver. However, you will not have to upload a physical.
-If you HAVE NOT played a sport this school year you will still need to complete this waiver. You will also have to upload the 2024-25 AIA physical to this waiver to be eligible to participate. The physical must be on the 2024-25 AIA physical forms to be able to be accepted. This will be valid for the next school year. We do not accept paper copies and the 8 pages must be uploaded. Hold onto this physical after uploading.
